Особенности ведения пациентов пожилого и старческого возраста с онкологическими заболеваниями. Консенсус экспертов в области гериатрии и онкологии

Старение является одним из факторов риска развития онкологических заболеваний — около 60 % больных раком в мире старше 65 лет. Планирование терапии с учетом гериатрического статуса и предпочтений пациента позволяет добиться лучших результатов и удовлетворенности больных. Проведение комплексной гериатрической оценки и разработка индивидуального плана ведения на этапе подготовки и лечения онкологического заболевания, а также после оперативного вмешательства, при проведении лучевой  и/или химиотерапии, способствует снижению риска осложнений и поддержанию качества жизни пациента. Должное внимание следует уделять инструментам преабилитации, в основе которой лежит улучшение физического состояния пациента, оптимизация питания, когнитивные вмешательства, снижение стресса и тревоги, коррекция анемии и других сопутствующих состояний. Меры, включаемые в преабилитационные программы, имеют синергический эффект: вмешательства, которые по отдельности могут не иметь значимого клинического результата, в комплексе дают значительное улучшение в послеоперационном периоде. Недостаточное количество клинических исследований у онкологических пациентов в возрасте 75 лет и старше является распространенной проблемой во всем мире, следует обозначить важность комплексного мультидисциплинарного подхода в ведении таких пациентов, чтобы улучшить общую выживаемость, качество жизни и снизить риск потенциальных осложнений.
