Повышение приверженности лечению у пациентов пожилого возраста с артериальной гипертензией (АГ) остается важной задачей современной кардиологии. Одним из перспективных подходов к ее решению является использование технологий дистанционного мониторинга.</p></sec><sec><title>ЦЕЛЬ ИССЛЕДОВАНИЯ</title><p>ЦЕЛЬ ИССЛЕДОВАНИЯ. Целью данного исследования было изучение возможности использования дистанционного мониторинга и оценка его влияния на приверженность измерению артериального давления (АД) и передачи данных с помощью чат-бота у пациентов с АГ в возрасте старше 65 лет.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В открытое рандомизированное контролируемое исследование включено 95 пациентов в возрасте ≥65 лет, которые были рандомизированы в 2 группы: 1) удаленного наблюдения (УН) (n = 50; медиана возраста 70 лет; 27 % – мужчины) и 2) контрольную (n = 45; медиана возраста 71 год; 40 % – мужчины). В группе УН применяли дистанционный мониторинг, который включал ежедневный опрос пациентов с помощью чат-бота в мессенджере. В случае снижения систолического артериального давления (САД) &lt;100 мм рт. ст. также предлагался дополнительный опросник для оценки симптомной гипотонии. В группе контроля пациенты наблюдались амбулаторно без использования удаленного мониторинга. Период наблюдения составил 3 месяца. Критерии оценки эффективности вмешательства: приверженность к заполнению чат-бота (оценивалась путем подсчитывания количества заполненных опросников за все время наблюдения) и уровень САД.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Через 3 месяца исследование завершили 45 пациентов в группе УН и 45 пациентов в контрольной группе. Повышение САД &gt;150 мм рт. ст. при домашнем измерении было достоверно чаще в группе контроля (группа УН 6 (13 %); группа контроля 20 (44 %); p = 0,001). Абсолютное снижение САД в обеих группах было статистически значимым. В группе УН абсолютное снижение САД составило 7,5 мм рт. ст. (p &lt; 0,001), в группе контроля – 5 мм рт. ст. (p = 0,003). Приверженность чат-боту и передача данных АД более 50 % времени наблюдения была у 43 пациентов (95 %), а более 75 % – у 27 (60 %). В группе УН симптомная гипотония наблюдалась у 22 человек (49 %).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Цифровые платформы для удаленного наблюдения могут быть эффективны в контроле АГ, способствуя повышению вовлеченности в процесс лечения пациентов старше 65 лет. Приверженность пациентов более 50 % времени наблюдения через мессенджер составила 95 %.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND. Improving treatment adherence in elderly patients with hypertension remains an important challenge in modern cardiology. One promising approach is the use of remote monitoring technologies.</p></sec><sec><title>OBJECTIVE</title><p>OBJECTIVE. The objective of this study was to evaluate the feasibility of remote monitoring and its impact on adherence to blood pressure measurement and data transmission via a chatbot in patients with hypertension over 65 years of age.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. This open-label, randomized, controlled trial included 95 patients aged ≥65 years who were randomized into 2 groups: 1) remote monitoring (RM) (n = 50; median age 70 years; 27 % men) and 2) control (n = 45; median age 71 years; 40 % men). The RM group utilized remote monitoring, which included daily patient surveys via a chatbot in a messenger. In case of systolic blood pressure (SBP) decrease &lt;100 mm Hg, a questionnaire to assess symptomatic hypotension was also offered. In the control group, patients were observed on an outpatient basis without the use of remote monitoring. The observation period was 3 months. The criteria for assessing the effectiveness of the intervention were adherence to filling out the chatbot (assessed by counting the number of completed questionnaires during the entire observation period) and SBP level.</p></sec><sec><title>RESULTS</title><p>RESULTS. After 3 months, 45 patients in  the UN  group and 45  patients in  the control group completed the study. An increase in SBP above 150 mm Hg during home measurement was significantly more frequent in the control group (UN 6 (13 %); control group 20 (44 %); p = 0.001). The absolute reduction in SBP in both groups was statistically significant. In the UN group, the absolute reduction in SBP was 7.5 mm Hg (p &lt; 0.001), in the control group 5 mm Hg (p = 0.003). Adherence to the chatbot and blood pressure data transmission for more than 50 % of the observation time was observed in 43 patients (95 %), and more than 75 % in 27 patients (60 %). Symptomatic hypotension was observed in 22 patients (49 %) in the remote monitoring group.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Digital platforms for remote monitoring can be effective in hypertension management, promoting increased engagement in the treatment process in patients over 65 years of age.
