Изучение синдрома старческой астении (СА) у больных фибрилляцией предсердий (ФП) крайне актуально в связи с высокой распространенностью данной аритмии, тесной связью ее с сердечно-сосудистыми и соматическими заболеваниями, а также увеличением с возрастом частоты инсультов и кровотечений.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить влияние СА на риск развития суммы прогноз-определяющих событий (тромбоэмболических и геморрагических осложнений) у больных ФП ≥ 60 лет, получающих антикоагулянты.</p></sec><sec><title>МАТЕРИАЛЫ И  МЕТОДЫ</title><p>МАТЕРИАЛЫ И  МЕТОДЫ. В  настоящий фрагмент из  одноцентрового проспективного регистра РЕГАТА-2 (РЕГистр длительной Антитромботической ТерАпии  — NCT043447187) было включено 455  пациентов ФП ≥ 60 лет, для которых доступно было оценить признаки СА по трем шкалам: «Возраст не помеха», Groningen Frailty Index и FRAIL. На протяжении наблюдения (медиана 6 лет) фиксировались прогноз-определяющие негативные события: тромбоэмболические и геморрагические осложнения BARC 2–5.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Признаки СА, по данным шкал Groningen Frailty Index и FRAIL, были выявлены у 29,7% и 31,8%, при использовании шкалы «Возраст не помеха» — у 9,5% пациентов. Наиболее часто встречающимися синдромами были эмоциональные и когнитивные расстройства, усталость и ограничение мобильности. По данным ROC-анализа, все три использованные шкалы имели предсказательную ценность в отношении риска суммы тромбоэмболических и геморрагических осложнений. На основании модели пропорциональных рисков Кокса определено, что независимыми предикторами прогноз-определяющих событий показали себя признаки старческой астении, определенные по шкалам Groningen Frailty Index (ОР = 1,8) и «Возраст не помеха» (ОР = 1,6).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Признаки старческой астении у больных фибрилляцией предсердий ≥ 60 лет повышают риск развития прогноз-определяющих событий, что обуславливает необходимость выделения этих пациентов в отдельную категорию для персонифицированного регламента ведения и наблюдения.</p></sec></abstract><trans-abstract xml:lang="en"><p>Studying the frailty in  patients with atrial fibrillation (AF) is  essential given the high prevalence of  AF and the escalating risk of strokes and bleeding as individuals age.</p><sec><title>AIM</title><p>AIM: to evaluate the effect of frailty on the risk of the sum of thromboembolic events and bleedings in patients with AF aged 60 years and older receiving anticoagulants.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. This fragment from the single-center prospective REGATTA-2 register (Register of  long-term Antithrombotic therapy -NCT043447187) included 455 patients with AF  ≥ 60 years for whom it  was possible to assess the signs of frailty on three scales Groningen Frailty Index, FRAIL and «Age is not a hindrance». During the follow-up (median of 6 years), the sum of thromboembolic events and bleedings were taken into.</p></sec><sec><title>RESULTS</title><p>RESULTS. Frailty according to the Groningen Frailty Index and FRAIL scales were detected in 29.7% and 31.8%, using the «Age is not a hindrance» scale — in 9.5% of patients. The most common syndromes were emotional and cognitive disorders, fatigue and limited mobility. According to  the ROC analysis, all three scales used had predictive value in relation to the risk of the sum of thromboembolic events and bleedings. Independent predictors (Cox proportional risks model) of sum of thromboembolic events and bleedings are Groningen Frailty Index ≥ 4 (HR=1.8) and «Age is not a hindrance» ≥ 3 (HR=1.6).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION.
