Витамин D играет важную роль в сохранении и поддержании функции скелетных мышц. Однако данные по применению колекальциферола с целью увеличения мышечной массы и улучшения функции скелетных мышц носят противоречивый характер.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить влияние нативных форм витамина D3 на мышечную силу и физическую работоспособность у пациентов с саркопенией, проживающих самостоятельно.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В проспективноеисследование включены 202 человека 65 лет и старше (медиана возраста — 75 [68; 79] лет), обратившихся за консультативной помощью в медицинское учреждение СанктПетербурга. Саркопению диагностировали в соответствии с критериям Европейской рабочей группы по изучению саркопении 2-го пересмотра (EWGSOP2, 2018). Концентрацию 25(ОН)D определяли на этапе скрининга, через 3 и 12 месяцев наблюдения. Эффективность колекальциферолау пациентов с саркопенией с применением разных режимов дозирования оценивали через 12 месяцев наблюдения.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. У пациентов с саркопенией дефицит витамина D встречался чаще по сравнению с лицами без саркопении (72,7% против 53,0%, р = 0,007) и ассоциировался со слабой мышечной силой (ОШ = 2,613; 95% ДИ 1,993–3,270; р = 0,00116), а также низкой физической работоспособностью по результатам SPPB тестов (ОШ = 1,905; 95% ДИ 1,342–2,710; р = 0,00034) и теста «Встань и иди» (ОШ = 1,364; 95% ДИ 1,003–1,853; р = 0,047). Прием колекальциферола вне зависимости от режима дозирования способствовал достижению адекватного уровня  25(ОН)D в сыворотке крови через 3 месяца у 87,5–98,5% людей (р &lt; 0,001) и сохранялся у 83,3–97,1% при повторном исследовании через 12 месяцев лечения. На фоне приема нативных форм витамина D3 у 82,8% пациентов с саркопенией увеличилась мышечная сила (р &lt; 0,001) и у 69,0% улучшилась физическая работоспособность (р &lt; 0,001).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Прием нативных форм витамина D3 способствует увеличению мышечной силы и улучшению физической работоспособности у пациентов с саркопенией.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND. Vitamin D influences the preservation and maintenance of skeletal muscle function. However, the effectiveness of cholecalciferol in enhancing muscle mass and improving skeletal muscle function has been the subject of varied opinions in existing studies.</p></sec><sec><title>AIM</title><p>AIM. This study aims to evaluate the effects of vitamin D3 supplementation on muscle strength and physical performance in community-dwelling patients diagnosed with sarcopenia.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. The research included 202 subjects aged 65 years and older (median age — 75 [68; 79] years) who were examined at a medical institution in St. Petersburg. Sarcopenia was diagnosed based on the European Working Group on Sarcopenia 2 revision (EWGSOP2, 2018) criteria. Levels of 25(OH)D were measured at the screening phase and again after 3 and 12 months of follow-up among those who consented to continue their participation. The effectiveness vitamin D3 supplementation was evaluated using various dosing regimens after 12 months.</p></sec><sec><title>RESULTS</title><p>RESULTS. Vitamin D deficiency was found to be significantly more prevalent in sarcopenic patients compared to their non-sarcopenic counterparts (72.7 vs. 53.0%, p = 0.007). This deficiency was associated with decreased muscle strength (OR = 2.613; 95% CI 1.993–3.270; p = 0.00116) and impaired physical performance, as indicated by the SPPB tests (OR = 1.905; 95% CI 1.342–2.710; p = 0.00034) and the «Timed Up and Go» test (OR = 1.364; 95% CI 1.003–1.853; p = 0.047). Following 3 months of native vitamin D3 supplementation with various dosing regimens, 87.5% to 98.5% of patients experienced an increase in 25(OH)D levels exceeding 30 ng/ml (p &lt; 0.001), with 83.3% to 97.1% maintaining these levels at the 12-month mark. After one year of treatment, muscle strength improved in 82.8% of sarcopenic patients (p &lt; 0.001), while 69.0% showed enhancements in physical performance (p &lt; 0.001).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION.
