В настоящее время растет число пациентов с хронической сердечной недостаточностью (ХСН), имеющих когнитивные нарушения (КН). В связи с этим появляется необходимость скрининга КН у пациентов с ХСН. Тест Мини-Ког является одним из скрининговых тестов, однако необходимы дополнительные исследования возможности применения данного теста на когорте кардиологических пациентов. </p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить чувствительность и специфичность теста Мини-Ког в выявлении пациентов с КН среди пациентов с ХСН 65 лет и старше.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. На базе обособленного структурного подразделения Российского геронтологического научно-клинического центра ФГАУ ВО РНИМУ им. Н.И. Пирогова с марта 2021 года по март 2023 года было отобрано 149 человек 65 лет и старше с ХСН. Когнитивный статус оценивался при помощи Монреальской шкалы оценки когнитивных функций, краткой шкалы оценки психического состояния. Всем пациентам был выполнен скрининговый тест Мини-Ког. Статистический анализ проводился при помощи ROC-анализа.</p></sec><sec><title>Результаты и выводы</title><p>Результаты и выводы. Частота встречаемости КН среди пациентов с ХСН 65 лет и старше по результатам нашего исследования достигает 67,8%. Отрезное значение 2 и менее баллов по тесту Мини-Ког (AUC 0,856; ДИ 95% 0,775–0,936, р &lt; 0,001) свидетельствует о наличии выраженных когнитивных расстройств с чувствительностью 61,5% и специфичностью 92,1%. Результат 3 и менее баллов (AUC 0,828; ДИ 95% 0,762–0,894, р &lt; 0,001) указывает на наличие УКР с чувствительностью 55,4% и специфичностью 93,7%, а деменции — с чувствительностью 80,8% и специфичностью 69,1%.</p></sec></abstract><trans-abstract xml:lang="en"><p>Currently, the number of patients with heart failure (HF) and cognitive impairment (CI) is growing. In this regard, it is necessary to screen for CI in patients with HF. The Mini-Сog test is one of the screening tests, but more research is needed to examine the feasibility of using it on a cohort of cardiac patients.</p><sec><title>Aim of the study</title><p>Aim of the study. The aim of the study is to assess the sensitivity and specificity of the Mini-Сog test in identifying patients with CI among patients aged 65 and over with HF.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From March 2021 to March 2023, 149 people aged 65 and older with chronic heart failure</p><p>(CHF) were selected from a separate structural unit of the Russian Gerontology Research and Clinical Center of the Pirogov Russian National Research Medical University. Cognitive status was assessed using the Montreal Cognitive Assessment Scale and the Mini-Mental State Examination. All patients underwent the Mini-cog testing. Statistical analysis was performed using ROC analysis.</p></sec><sec><title>Results and conclusions</title><p>Results and conclusions. The prevalence of cognitive impairment among patients with CHF aged 65 or older, according to our study, is 67.8%. A cutoff value of 2 points or less points on the Mini-Cog test (AUC 0.856; CI 95% 0.7750.936, p &lt; 0.001) indicates the presence of severe cognitive impairment with a sensitivity of 61.5% and a specificity of 92.1%.
