Результаты последних фундаментальных и клинических исследований указывают на важную роль инсулиноподобного фактора роста 1 (ИФР-1) в процессе старения. При этом в основе снижения функциональных и адаптационных возможностей, сопровождающего процесс старения, лежит развитие гериатрических синдромов. На данный момент нет единого мнения о вкладе возраст-ассоциированного снижения секреции ИФР-1 в развитие этих синдромов, что обуславливает актуальность нашего исследования на уникальной когорте долгожителей.</p></sec><sec><title>ЦЕЛЬ ИССЛЕДОВАНИЯ</title><p>ЦЕЛЬ ИССЛЕДОВАНИЯ. Изучение связи уровня ИФР-1 с гериатрическими синдромами у долгожителей.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В рамках поперечного исследования была изучена когорта долгожителей в возрасте 90 лет и старше, для которых проводилась комплексная гериатрическая оценка со сбором анамнеза, применением гериатрических шкал и опросников. Уровень ИФР-1 в сыворотке крови измерялся с помощью хемилюминесцентного анализа. Исследование было одобрено локальным этическим комитетом (протокол № 30 от 24.12.2019). Статистический анализ и визуализация данных выполнялись с помощью языка программирования R версии 4.2.2.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В исследование были включены 3789 человек в возрасте от 90 до 107 лет, соответствующих критериям включения. Медиана возраста составила 92 года, медиана уровня ИФР-1 — 99,8 нг/мг (min — 30,8 нг/мл, max — 208 нг/мл). По результатам межгруппового сравнения значимую связь с уровнем ИФР-1 показали зависимость от посторонней помощи в повседневной жизни, старческая астения, высокий риск падений, когнитивные нарушения, саркопения и мальнутриция. Однако после введения поправок на физическую активность и индекс массы тела статистическая значимость сохранилась только для ассоциации уровня ИФР-1 с когнитивными нарушениями.</p></sec><sec><title>ВЫВОДЫ</title><p>ВЫВОДЫ. Полученные нами результаты позволяют предположить, что снижение уровня ИФР-1 повышает вероятность развития когнитивных нарушений у долгожителей. Тем не менее требуется дальнейший анализ подобных ассоциаций в проспективных исследованиях.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND. The role of insulin-like growth factor-1 (IGF-1) and its levels in aging have been established in clinical and experimental studies. Aging-associated geriatric syndromes underlie functional and adaptive decline in older patients. However, there is insufficient evidence that aging-associated decrease in circulating levels of IGF-1 is associated with geriatric syndromes, particularly in long-living adults. This study aimed to examine the associations between IGF-1 levels and geriatric syndromes in long-living adults.</p></sec><sec><title>METHODS</title><p>METHODS. This cross-sectional study included nonagenarians recruited with the assistance of social services, nursing homes, geriatric centers, and other geriatric services. The participants underwent a comprehensive geriatric assessment, including medical history, geriatric scales, and questionnaires. Blood samples were collected, and IGF-1 levels were assessed using chemiluminescence. The study was approved by the ethics committee (Protocol № 30 dated December 24, 2019). Statistical analysis was performed using the R package (v. 4.2.2).</p></sec><sec><title>RESULTS</title><p>RESULTS. The study included 3,789 participants aged 90–107 (median age = 92) who met the inclusion criteria. The median IGF-1 level was 99.8 ng/mg (min: 30.8 ng/ml; max: 208 ng/ml). The intergroup analysis showed associations between IGF-1 levels and functional dependence, frailty, risk of falls, malnutrition, sarcopenia, and cognitive impairment. However, after adjusting for physical activity and BMI, a significant association was demonstrated only between IGF-1 levels and cognitive impairment.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Our study showed a significant association between IGF-1 levels and cognitive impairment.
