В настоящее время не существует единого подхода к стратификации риска у пациентов старческого возраста перед кардиохирургическим вмешательством. Известные способы прогнозирования исходов аортокоронарного шунтирования не учитывают особенности пациентов старшей возрастной группы.</p></sec><sec><title>Цель</title><p>Цель. Создание шкалы прогнозирования ранних послеоперационных исходов при аортокоронарном шунтировании у пациентов старше 75 лет на основании данных комплексной гериатрической оценки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное исследование были включены 83 пациента старше 75 лет, имеющие показания к реваскуляризации миокарда. Критерием исключения явились показания к экстренному оперативному вмешательству. Всем пациентам помимо стандартного обследования была выполнена комплексная гериатрическая оценка, включающая скрининг на наличие старческой астении, оценку наличия когнитивных нарушений, диагностику депрессии, динамометрию и проведение краткой батареи тестов физического функционирования (определение скорости ходьбы, времени подъема со стула и равновесия).</p></sec><sec><title>Результаты</title><p>Результаты. Были разработаны 6 математических моделей, прогнозирующих следующие исходы раннего послеоперационного периода аортокоронарного шунтирования: длительность искусственной вентиляции легких (р = 0,012), время пребывания в реанимации (р &lt; 0,001), длительность постельного режима после операции  (р &lt; 0,001), длительность инотропной поддержки (р &lt; 0,001), риск возникновения острого нарушения мозгового кровообращения (р = 0,015), длительность койко-дня после оперативного вмешательства (р = 0,003). Модели были внесены в программу Microsoft Excel, полученный способ прогнозирования назван «Гериатрическая прогностическая шкала». Для расчета риска по данной шкале необходимо ввести данные: скорость ходьбы (м/с), динамометрия (кг), время подъема со стула (сек), равновесие (сек), общий балл краткой батареи тестов физического функционирования (балл), общая фракция выброса по методу Simpson (%).</p></sec><sec><title>Заключение</title><p>Заключение. Разработанный способ дает возможность прогнозировать ранние исходы аортокоронарного шунтирования больных старше 75 лет в зависимости от их функционального статуса, что является значимым дополнением к оценке по стандартным шкалам риска, не учитывающим особенности пожилых пациентов. Способ простой в исполнении и может быть выполнен самостоятельно лечащим врачом пациента.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Currently, there is no standardized method for assessing risk before cardiac surgery in older individuals. Recognized operative risk scales do not consider the features of older patients.</p></sec><sec><title>Aim</title><p>Aim. To create a scale that can forecast early postoperative outcomes after coronary artery bypass grafting in patients aged over 75 by conducting a comprehensive geriatric assessment.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study enrolled 83 patients aged 75 and older who required myocardial revascularization, with emergency surgery cases as the exclusion criteria. In addition to the standard screening, all patients underwent comprehensive geriatric assessment, including frailty screening, cognitive assessment, depression screening, grip strength, and short physical performance battery test (walking speed, sit-to-stand performance, standing balance).</p></sec><sec><title>Results</title><p>Results. Six mathematical models predicting coronary artery bypass grafting outcomes were developed: duration of mechanical ventilation (р=0,012), length of stay in intensive care unit (р&lt;0,001), duration of bed rest after surgery (р&lt;0,001), duration of inotropic support (р&lt;0,001), risk of acute cerebrovascular accident (р=0,015), postoperative length of stay (р=0,003). Within Microsoft Excel, mathematical models were used to devise the «Geriatric prognostic scale» for anticipating early postoperative outcomes post coronary artery bypass grafting. To estimate the risk, the following data must be entered: walking speed (m/s), grip strength (kg), sit-to-stand performance (sec), standing balance (sec), total score on the Brief Physical Functioning Test Battery, and total ejection fraction using the Simpson method (%).</p></sec><sec><title>Conclusion</title><p>Conclusion. The developed method allows for the early prediction of outcomes in coronary artery bypass grafting for patients over 75 based on functional status. This is a significant addition to traditional risk scales, which do not consider the features of older patients.
